ACHIEVE JIU JITSU

Release of Liability, Waiver of Claims, Assumption of Risk, and Indemnity Agreement

This Release of Liability (“Agreement”) is entered into between Achieve Jiu Jitsu (“Academy”) and the undersigned participant (“Participant”), or the parent/legal guardian if the Participant is under 18 years old.

1. Acknowledgment of Risks

I understand and acknowledge that Brazilian Jiu Jitsu, grappling, martial arts training, and related physical activities involve inherent risks, including but not limited to:

• Physical injury (including sprains, strains, fractures, concussions, or other trauma)
• Cuts, bruises, abrasions, and joint or muscle injuries
• Serious injury or death
• Risks associated with training with partners of varying sizes, skill levels, and physical conditions
• Exposure to illness, including communicable diseases

I voluntarily choose to participate in these activities with full knowledge of these risks.

2. Assumption of Risk

I freely and voluntarily assume all risks, known or unknown, associated with participation at Achieve Jiu Jitsu, including risks arising from negligence by instructors, staff, or other participants, to the fullest extent allowed by law.

3. Release of Liability

To the fullest extent permitted by law, I hereby release, waive, and discharge Achieve Jiu Jitsu, its owners, instructors, employees, volunteers, agents, representatives, and affiliates from any and all claims, demands, causes of action, damages, losses, costs, or liabilities arising out of or related to my participation in Academy activities.

4. Indemnification

I agree to defend, indemnify, and hold harmless Achieve Jiu Jitsu and its staff from any claims, demands, or liabilities brought by third parties arising out of my actions or participation in training.

5. Medical Treatment

I authorize Achieve Jiu Jitsu staff to secure emergency medical care if deemed necessary. I understand I am solely responsible for all associated costs.

6. Health Certification

I affirm that I am in appropriate physical condition to engage in Jiu Jitsu and related training. I have no known medical conditions that would increase my risk of injury and have consulted a physician if necessary.

7. Personal Belongings

I acknowledge that the Academy is not responsible for lost, stolen, or damaged personal property.

8. Media Release (Optional)

I grant permission for Achieve Jiu Jitsu to use photos or videos of me taken during classes or events for promotional or educational purposes.

9. Minors

If the Participant is under 18, the parent/guardian must sign below and agrees to all terms on behalf of the minor.
